**THIS ADVERT IS FOR A MARKET ENGAGEMENT EVENT** Herts Valley Clinical Commissioning Group (HVCCG) is holding a market engagement event for providers who are interested in tendering for the provision of Adult Community Services in west Hertfordshire. Herts Valley CCG comprises of 67 GP Practices arranged over four geographical localities. HVCCG is responsible and accountable for PS885 million of NHS care for the 620,000 population of west Hertfordshire. Herts Valley Adult Community Services will comprise of the following main features: * Easy access to adult community health services through a single point of access * Specialist input in the community that is integrated with the enhanced and core teams * Localised integrated care that enables complex case management * Prevention of Admission and supported discharge services - Facilitated seamless flow between planned and unplanned care * Outcome based * Promotes health and well being The service is to be delivered on behalf of all Herts Valleys over 18 years registered patients and is expected to start on the 1st April 2019. It is anticipated that the formal procurement process will begin in March 2018. The proposed Adult Community Health Services included are: * Enhanced and Core Integrated Community Nursing and Therapy Services * Intermediate Care Community Beds * Bladder and Bowel * Adult Community Speech and Language Services * Cardiac Rehabilitation * Community Neuro Rehabilitation Service * Lymphodema * Tissue Viability Service * Specialist Palliative Care * Podiatry This event will offer potential providers an opportunity to hear about Herts Valley CCG's strategy and vision for Adult Community Health Services. This is an opportunity for you to hear more about our proposals, provide feedback and to ask us questions, as well as an opportunity for potential providers to network, forge relationships and influence service delivery models. The event will be held on the 28th February 2018 and will commence at 2.00pm and is expected to end at 5.00pm. The location will be provided upon registration. Tea and Coffee will be provided. Places are strictly limited therefore reservations are restricted to no more than 2 persons per organisation.
