Notice Description

Surrey County Council is looking to procure a number of Collaborative Reablement Providers for their new Collaborative Reablement Service. The Service can be briefly described as one where providers deliver a reablement service in partnership with Surrey County Council's Reablement Teams. Surrey County Council's Reablement Service can be described as a short term service delivered within the clients own home for up to 6 weeks focusing on re learning or developing new skills, regaining confidence and utilising personal strengths to maximise independence. The Service responds to referrals from both the hospitals and also from the community teams - endeavouring to work preventatively, avoid unnecessary admissions and enable clients to remain safe and independent at home for longer. Reablement forms one of the tertiary care services, alongside short-term crisis response services, intermediate care services delivered by health professionals and bed-based intermediate care services. It is a short term (up to six weeks) intervention following hospital discharge or after an emergency at home, the aim of which is to help people to learn to accommodate and manage their illness or condition and maximise their level of independence by learning or relearning skills necessary for daily living. Reablement services work closely with other services and disciplines as part of a multi-disciplinary team and is a non-chargeable service for anyone who is able to benefit from it. It is important to note that the underpinning ethos behind reablement is different to traditional home-based care - instead of solely supporting people with personal care tasks or meal preparation, reablement is also focused on providing support, encouragement and building individuals' confidence with all aspects of daily living. Reablement workers encourage individuals to regain or re-learn their ability to do things for themselves and draw on their strengths, focusing on maximising people's independence. In so doing they help people to define their own goals, so that positive outcomes are achieved for both the individuals being supported and the wider social care and health system. Reablement is regarded as a crucial intervention for a sustainable health and social care system, not only by supporting more people to live independently for longer but, in turn, reducing any unnecessary reliance on care resources and the risks of future care emergencies. Despite this, the NHS Long Term Plan estimates that, of the people currently discharged from hospital whose best outcome would be to receive reablement support, fewer than 50% actually receive it2. It is therefore vital for local authorities to maximise the impact of reablement in their area by expanding the service and offering it to everyone who needs it in their areas.
