Notice Description

Our vision for a new integrated wellness service is to reduce health inequalities through better service integration, moving resources away from avoidable treatment and care and towards prevention and early intervention. An approach to targeting resources at individuals and communities of greatest need will be required. As an incentive to providers this specification and subsequent contract includes an Equity Premium payment designed to reward the provider for increasing the uptake of the service and completion of behaviour change by people living in areas of deprivation. Public Health intends to commission services in an integrated model, to: * Improve outcomes and quality of service delivery for residents place a greater emphasis on prevention and early intervention by promoting health and wellbeing rather than on diagnosing and treating illness. * Empower individuals to maintain and improve their own health and remain independent for as long as possible. * Apply the principles of a strength based approach using the local resources, skills and opportunities which individuals and communities have at their disposal. Public Health is seeking to commission a new service that will integrate previously contracted multiple lifestyle and wellbeing services. The new integrated service seeks to contribute to achieving the five aims of the Rotherham 2015 - 18 joint Health and Wellbeing Strategy which identifies that the key aims of the strategy can be tackled by a 'whole system' approach, and needs the involvement of the whole health and care system to achieve improvement. The five aims of the Health and Wellbeing Strategy are : * All children get the best start in life * Children and young people achieve their potential and have a healthy adolescence and early adulthood * All Rotherham people enjoy the best possible mental health and wellbeing and have a good quality of life * Healthy life expectancy is improved for all Rotherham people and the gap in life expectancy is reducing * Rotherham has healthy, safe and sustainable communities and places In order to improve the public health needs of the people in Rotherham, it is felt a more holistic approach is necessary which takes into account the resources that are readily available and that will help enable people to sustain healthier living well as delivering a new integrated wellbeing services. Therefore the following existing services are being commissioned in a single service offer: * NHS Health Checks * Adult Weight management * A population alcohol risk screening intervention * The health trainer service (see Appendix F for Current Rotherham Health Trainer Service Description) * Support to stop smoking The above services combined with a new model of tiered delivery based on need, using innovative methods of access and delivery including a digital transformation of services, will form the core offer of the new service.
