From April 2017 to March 2020 Stoke-on-Trent City Council will deliver a group based intervention for high risk adults. This tender seeks to appoint a Provider to supply a three-year licence which will enable delivery of group based intervention for adults in a community setting within Stoke-on-Trent. The model should be evidence based, underpinned by established behaviour change theories with a positive impact on clinical outcomes which must include Body Mass Index. This model or intervention should reflect relevant NICE Guidance.
