ASC Independence at Home - Soft Market Questionnaire
Notice Description
ASC Independence at Home - Soft Market Questionnaire The Independence at Home Service is commissioned to deliver hands on, responsive, short-term, practical, compassionate, and personalised support to enable a person, wherever safely possible, to remain at, or return to their own home following discharge. The support is aimed to avoid long-term reliance, by linking individuals into existing support structures and community assets within a 6 week period. The service does not deliver any personal care. The service will also prevent, reduce, or delay a need for longer term health or care services by preventing unnecessary hospital admission relating to lack of home support and enabling safe and timely discharge from hospital. The service is accessed via referral from a health and care professional to the commissioned provider who co-ordinate the response via paid staff and volunteers. There is also the option to self-refer. QUESTIONNAIRE: THIS IS NOT A CALL FOR COMPETITION Please answer the following questions and send your response back via email to ckesterton@worcestershire.gov.uk by 4pm on 7th July 2023. You can also find them in the attached Word document you can then complete and return. All responses will be treated as confidential and not shared with other organisations, however, the final specification and tender model may be influenced by this and other stakeholders. 1) Are there any areas that you think have the potential for improvement / efficiency, and why? 2) Is there anything you think the Council needs to consider or do in terms of commissioning this service to ensure we deliver the maximum impact within the available resources? 3) What length of contract would your organisation be looking for? What are the drivers for this? 4) Do you have any thoughts on delivering social value through the contract? Please list possible benefits which may form part of the contract.
