Notice Description

The City Council and Western Locality of the NEW Devon Clinical Commissioning Group are developing a network of Wellbeing Hubs across Plymouth. This includes 10 'targeted' or building based hubs. The main aim of the Hubs will be to support the transformation of models of care to improve health and wellbeing outcomes and reduce health inequalities whilst reducing pressure on acute, secondary and primary care settings. We are holding some engagement sessions with providers and the wider market to discuss how we might shape service provision in future to achieve the new models of care in the hubs. There will be two separate discussions: 1. The future shape of service provision - Friday 6th July, 10am to 1pm 2. How services might interact with each other to deliver the support required in the hubs - Monday 23rd July, 10am to 1pm PLEASE NOTE: THESE ARE TWO DIFFERENT ENGAGEMENT EVENTS, YOU MAY WISH TO ATTEND BOTH
