Notice Description

NHS North Lincolnshire Clinical Commissioning Group (CCG) would like to engage with providers of residential and/or nursing care in the area, to determine interest in the supply of community based intermediate care beds. The CCG currently spot purchases a number of beds across the area, but we are looking to implement a more strategic approach to this provision. While 'Home First' is always the priority outcome for local people leaving hospital, community beds are an important component of the wider intermediate care model; providing an alternative to a hospital admission, as well as to help move people out of hospital quickly and complete any rehabilitation to support people's future independence in their own home. We would like to engage with Providers, prior to any possible future procurement, so that we can assess the local market and determine the appropriate model of delivery. We are looking for providers who will become part of the wider health and social care system - contributing to reducing pressures on our acute hospital services and helping patient flow through the system. As such we are exploring how we might commission both the bed provision and the lower level rehabilitation support to help people recover and return home; ensuring that wider packages of care are coordinated and in place. Key Outcomes 1. Contribute to the timely discharge from hospital, when the home first ethos has been explored and deemed inappropriate due to safety and/or level of care needs. 2. Reduce the level of long term care packages, by facilitating a period of additional care and support until the patients' health needs are resolved and provide opportunity to avoid long term residential care. 3. Ensure people are moved through community intermediate care beds quickly with appropriate future support packages in place as needed. 4. Staff trained in rehabilitation and reablement and working as part of the wider health and social care system Please note that a market engagement event is to be held on: 29th June 1.30pm to 4pm.
