Keeping Well Services (Market Engagement Questionnaire)
Notice Description
The detail of the future Keeping Well Service model in Sunderland has not yet been determined, however it may include services to provide care and support to adults aged 18 years and over who are living in their own home, specifically adults who have care and support needs arising as a result of: * Frailty * a physical disability * a sensory impairment * a mental health need, including dementia/cognitive impairment. * a learning disability including autism and challenging behaviour. * substance misuse It is not intended that the services that may be included in the Keeping Well Service are specific reablement services, but to support the achievement of the three key priority areas in our Living Well and Ageing Well Strategy for Adult Social Care in Sunderland we will expect all care and support services to be delivered with a reablement ethos. The Council are undertaking a soft market test because: * We are looking to you to help inform the development of the services commissioned in Sunderland to improve outcomes for local people. * As a service provider, you have important insights into what will or won't work in service delivery. This is your opportunity to influence the development of the specification. Please note that any information you provide as part of the soft market test may be used to inform and influence the development of the service
