The Commissioner (Coventry City Council and Coventry & Rugby Clinical Commissioning Group) is looking to contract with a provider for the delivery of long-term home support, specifically for the Wyken, Binley & Willenhall and Lower Stoke wards of the City (Cluster 6). This will be for Adults and Children that meet national eligibility criteria for social care and Adults eligible for non-complex health funded Continuing Health Care at home. The service will provide personalised care and support to people in their own homes to enable individuals to live independently.
