CORNWALL COUNCIL - Soft Market Test/Early Engagement Extra Care is the provision of housing with care and support to enable people to develop or maintain their independence, minimise the risk of social exclusion and improve their quality of life, thus preventing loss of their home or tenancy and the otherwise unnecessary use of more institutional forms of care. Extra Care housing offers a way of supporting people to live independently with care and support available on the premises 24 hours a day. It gives security and privacy within people's own home (which can either be owned or rented) with a range of facilities and the option to join in with communal activities if they so wish. The Council is looking for an organisation/s to provide up to approximately 60 self-contained units (each unit would comprise separate bedroom/s, bathroom, and kitchen) of Extra Care accommodation in a scheme with the appropriate communal facilities (i.e. lounge, outside space, accessible toilets, treatment room) and lift access. The accommodation should be available to the Council by 1st April 2016. In addition, the required service provision will include, but not be limited to: * Promoting independence by supporting and encouraging service users to undertake their own self-care and daily living tasks as far as possible; * Support with personal care tasks; * Support with communication, social care and social integration; * Support to enable service users to meet their tenant or owner responsibilities to maintain their home, maintain safety and security, and access leisure activities; * Provision of domestic support including, but not limited to cleaning, laundry, shopping, whilst promoting the service users independence; * Support to access and provide services and activities that promote mental and physical stimulation and social networks within the local community for the benefit of service users and those living in the surrounding area who may benefit from these opportunities; * Support to reduce personal and social isolation by developing their own social network, encouraging links with family, friends and the wider community as appropriate; * Support with any other task that meets the service user's needs and outcomes which are identified in their support plan. The organisation/s must be registered with the Care Quality Commission in order to deliver all of the above services. The potential business value is estimated at PS0.5 million per annum.
