Provision of Patient Discharge Services
Notice Description
NHS Shared Business Services is looking to gauge market interest and conditions for the Provision of Patient Discharge Services for adults 18 years and above. The purpose of this Early Engagement Notice is to: * Share a vision and proposal on the way forward * Gather views and comments from a range of providers and interested parties. A delayed transfer of care is where a patient is ready and safe to leave hospital care, but is unable to do so, and remains occupying a hospital bed. Hospital discharge schemes are proven to significantly reduce delayed transfers of care (DTOC) and prevent "bed blocking". The main objectives of this proposed framework are; *Reduce the resource pressure on acute hospital beds with improved discharge services *Reduce pressure on Social Services *Reduce instances of re-admission to hospital *Discharge of patients meeting specific clinical criteria into a safe, high quality care environment 1. Domiciliary Care in your Own Home, (low level support, extended support or overnight stay) Take home and settle, this would include transport home and offers low level support to settle an individual back into their home Extended support on the first visit while they are getting settled back in, to include shopping for basic essentials, making up beds, and ensuring that the house is clean and tidy. This is essential for patients with little or no family support. 2. Bed Based Intermediate Care (or Step Down Beds, possibly in a residential home) The Service will offer rapid response, short-term home based care to people who are ready to be discharged from hospital and have been initially assessed as requiring a home care package. It will deliver support while longer term services are being organised. 3. Third party care brokerage (a service for Care Home selection and/or Domiciliary Care provider selection) Please complete the Pre-Procurement Questionnaire that is attached to this PIN Notice. Any responses to this early market engagement exercise imply no commitment on Providers to engage in any subsequent procurement process, nor do they confer any advantaged status or guarantee of inclusion in any subsequent procurement process for those Providers who do respond. The questionnaire and all responses received are in no way legally binding on any party. Please respond either through the EU Supply tender portal, tender reference 31200 or directly to Andrew Payne, andrew_payne@nhs.net by 12:00 noon on Wednesday 25th April 2018. Additional information: Please complete and return the Pre-Procurement Questionnaire that is attached to this PIN Notice. NHS Shared Business Services reserve the right to withdraw this notice at any time. NHS Shared Business Services is not bound to accept any proposals submitted by Providers and is not liable for any costs incurred as a result of Providers engaging with this process. This Early Market Engagement Exercise does not guarantee that procurement will take place and NHS Shared Business Services reserves the right to defer from any procurement entirely.
