The Commissioners are seeking to enable the integration of Carers services through a Carers Hub model to coordinate support for Adult Carers and Young Carers across Cheshire East providing one point of contact forCarers to coordinate and improve access to local support. The Commissioners require a Provider for the management, co-ordination and provision of an Integrated CarersHub for Adult Carers and Young Carers and their families.
