Notice Description

For a duration of Three plus one (3 + 1 years,extenstion) the Provider will provide 52 weeks per calendar year of physiotherapy advice and assistance on a Wednesday to Council employees who have been identified, and mutually agreed with that Council employee's manager and an occupational health staff (OHS), to have a condition that may benefit from physiotherapy. The Physiotherapist will treat employees for a maximum of six (6) weeks, or make forward referrals as required. After six weeks if further treatment is indicated the employee will be offered an additional three (3) weeks or sign post to more appropriate support. In addition, the Provider will: * ensure one single practitioner per individual treatment plan, supporting consistency of care, and * ensure adequate and consistent cover is provided for any sickness absence, annual leave or any other form of absence / leave taken by the practitioner.
