WYICB (Bradford District and Craven Health & Care Partnership) Universal Children's Sleep Support Model As part of the NHSE inequalities strategy funding "Core 20 plus 5" NHS England " Core20PLUS5 - An approach to reducing health inequalities for children and young people and the Bradford Adversity Trauma and Resilience program Bradford District and Craven Health and Care Partnership and BMDC are looking for a provider of a sleep support model for children (2-11yrs) and their families. Working as part of the districts "Act as One" approach to children and young people the service will be integrated in to the wider children and young people program and support the Bradford District Adversity Trauma and Resilience strategy Who we are - Bradford ATR (bradfordatrpartnership.co.uk) and will be embedded in the districts refreshed family hub model Home | Bradford Families and Young Persons. This service will support a universal sleep support offer for children aged 2-11 years. We will require a provider to: - Provide evidence based training and appropriate materials for the children and young people workforce offering first line support to families in Bradford district and Craven. This should be done in partnership with those already providing elements of training in the district. - Offer family and child centred group support in the family hubs and community venues ensuring consideration of ease of access for deprived communities. - Offer flexible one to one support sessions where needed offering a variety of ways to meet with families including outside of working hours - Support families with the development of sleep plans to promote the establishment of good sleep habits and patterns - Provide families with follow up support 12 weeks after creation of a sleep plan - Refer to Primary care and Secondary care as appropriate for medical investigation - Contribute to the development of a sleep pathway in Bradford working closely with colleagues in the infant mental health service and specialist services - Carry out a needs assessment of CYP aged 11-19yrs and consider the introduction of a model for young people working with the 0-19 public health nursing service and partners - Contribute to the reduction in Melatonin prescriptions currently being used by children aged 2-11yrs in Bradford District and Craven Additional information: If you would like to submit your interest for this service, please express your interest by registering on https://nhswyicb.ukp.app.jaggaer.com/ and complete the questions included in PQQ_37.
