Lincolnshire Wellbeing Hubs and Community Connector Services
Notice Description
NHS Lincolnshire CCG would like to invite providers to attend a virtual Market Engagement Event regarding the provision of Wellbeing Hubs and Community Connectors Services across Lincolnshire, which will form the basis of two future procurements. Due to the rurality and poor transport infrastructure of the East Lindsey, SOLAS and First Coastal Primary Care Networks the vision is to build a wellbeing hub (central co-ordination) based in each of the three PCN's which will be extended to cover the wider population via satellite spokes in towns and villages. The model is embryonic and provision should be based on local need, however a mixture of the following will be provided: *Arts &Crafts Groups *1:1 Befriending *Financial advice/employment support *Cafes/community drop ins The Crisis Resolution Home Treatment teams will promote the Mental Health Wellbeing hubs and use these venues as a place to 'meet' clients, and connect them to activities and resources in their community.
