Notice Description

NHS Lincolnshire CCG would like to invite providers to attend a virtual Market Engagement Event regarding the provision of Wellbeing Hubs and Community Connectors Services across Lincolnshire, which will form the basis of two future procurements. Due to the rurality and poor transport infrastructure of the East Lindsey, SOLAS and First Coastal Primary Care Networks the vision is to build a wellbeing hub (central co-ordination) based in each of the three PCN's which will be extended to cover the wider population via satellite spokes in towns and villages. The model is embryonic and provision should be based on local need, however a mixture of the following will be provided: *Arts &Crafts Groups *1:1 Befriending *Financial advice/employment support *Cafes/community drop ins The Crisis Resolution Home Treatment teams will promote the Mental Health Wellbeing hubs and use these venues as a place to 'meet' clients, and connect them to activities and resources in their community. Community Connector roles we be recruited to each area to lead local initiatives, they will be required to: - Support an alternative provision to 'crisis'/ that would be deemed appropriate for low level support by supporting people to access a safe, supportive and therapeutic environment, promoting independence, opportunity and recovery for all. - Develop community-led events and activities to engage and connect key communities and partners to projects, making best use of the available community assets and driving peer support for community groups and community leaders. - Support de-escalation and 'step down' from the 24/7 Lincolnshire Mental Health helpline and embed pathways to support escalation up for support. - Identify and establish linkages with key community leaders and individuals, to identify solutions, build skills, community activities and facilitate new connections. - Provide support and encouragement for communities to identify, map and promote their community assets and publish on the Lincolnshire County Council Connect 2 Support Directory if Services- - Build on the increased levels of social action and the willingness for people to support their communities - Ensure support reaches out to vulnerable, deprived and isolated members of society and identify ways to enhance access and reduce barriers. The initial contract length will be for a period of 18 months initially with a view to review and confirm future provision after this period. The CCG would like to invite all potential bidders to two events, so they can be informed of the detail of the service & procurements.
