Notice Description

The core vision is to deliver and continuously develop a high quality Urgent Care Home visiting service (HVS) which operates as part of a locally defined integrated Urgent care system that streamlines access for patients through the urgent and unscheduled care system. The Urgent Care Home Visiting service will offer urgent care at home service for housebound patients and an urgent primary care visiting service out of core hours. This service will therefore by no means replace the home visiting service element in the GP primary service contract. The following core principles reflect the ambition for the Integrated Urgent Care Service and as the service evolves these core principles are likely to develop further. * To provide a seamless and continuous service throughout the 24 hour period, so that the distinction between in hours and out of hours becomes redundant. * Connect urgent care services together more efficiently to reduce a fragmented and complex system * The Provider will support care home staff in building skill and confidence to manage patients with an urgent presentation. * To increase access to an appropriate urgent care Home Visiting Service for patients residing in care homes. * To reduce avoidable Emergency Department (ED) attendances and emergency admissions 24 hours 7 days a week period for patients, by providing appropriate clinical care through proactive, and timely health care intervention. * To improve sign posting to the appropriate clinical pathways (e.g. Proactive care, community nursing, social care) reducing duplication and waiting for patients * To develop relationships locally with Community service providers with a particular focus on GP practices. * Work in partnership to complement existing and future unplanned care services in LLR * Contribute towards a reduction in avoidable 999 ambulance conveyances * To support the patient to remain in their usual place of residence by supporting patients, carers and families through agreed care plans or help to self-manage * Enhance the quality of the patient experience. The Urgent Care Home Visiting Service (HVS) will provide a service that supports the urgent care system, with patients who have an urgent health needs and who are at increased risk of admission into hospital. The service aims to reduce the number of avoidable emergency admissions and attendances at ED and to maximise the use of other care pathways over a 24-hour cycle. The service will receive referrals from LLR Clinical Navigation hub, General practitioners' (GP) partners, and care home staff and the for patients experiencing acute episodes of illness or injury that are at high risk of attendance or admission to hospital. Additional information: There is the expectation that this provider will join the anticipated Leicester, Leicestershire and Rutland (LLR) Urgent Care Alliance.
