The service will work in partnership with health, social care, housing and education departments and agencies across County Durham to promote support services and referrals into relevant VCSE organisations when the need is identified. This will include but is not limited to , other commissioned services, as well as health and adult services such as Social Care Direct and Community Mental Health Teams
