The CCG invites interested organisations to express an interest and submit a duly completed Market Engagement Questionnaire by 12:00 on 30 October 2019. You must register an account on our e-tendering portal (https://procontract.due-north.com). Once registered and logged in, you must express an interest in the project to access the Market Engagement Questionnaire and the specification. Further instructions on how to access the project can be found at the following link: https://supplierhelp.due-north.com/Opportunities1.html Once in the project screen the market engagement questionnaire and specification can be accessed, completed and submitted via the messaging facility on the portal. The Market Engagement Questionnaire can be accessed via the e-tendering portal at https://procontract.due-north.com/Procurer/Advert/View?advertId=38f57ed1-1ce1-e911-80fa-005056b64545 Please note that these services come under 'Schedule 3 - Social and Other Specific Services' under the Public Contracts Regulations 2015 (the 'Regulations'), and Tower Hamlets CCG is bound only by those parts of the Regulations applying to Schedule 3 Services. Barnet CCG has published a Prior Information Notice (PIN) as per Regulations 75(1)(b)(iv). Please note that the contract/s will be awarded without further publication of a Contract Notice. Additional information: Barnet CCG would like to engage with potential providers with relevant expertise and experience to deliver a Home from Hospital Supported Discharge service. It is envisaged that the service will include supporting around 130 vulnerable patients to be discharged from Barnet and the Royal Free Hospitals and Edgware and Finchley Memorial Community Hospitals a month. Patients will be offered practical and emotional support in their homes and place of residence. Support will be of a one off nature for some patients and up to 4-6 weeks for others, depending on need. Social prescribing will form a key element of this service, and patients will be supported to connect with local voluntary and community services to help improve their wellbeing and social welfare. The service is aimed at improving patient experience within Barnet and the Royal Free Hospitals and Edgware and Finchley Memorial Community Hospitals. It will reduce anxiety by promoting people's independence. Post discharge, the service aims to enable patients to stay at or move back home, in safety and comfort by offering flexible support and dealing with practical issues to allow the person to settle back into a normal routine. Objectives of the service include: Reducing anxiety and improving patient experience within hospital. Providing a time limited but flexible practical support, e.g. carrying out small tasks, accompanying service users to appointments, emotional support and encouragement Promoting independence in managing daily living tasks Signposting people to other local statutory, community and voluntary services for on-going support. Short term bridging support while further statutory service input is organised Sitting service to provide respite to carers or free up time to perform other tasks The range of tasks that should be undertaken by the service includes: Reducing anxiety Accompany patients home upon discharge Key cutting Receive delivery of OT equipment in patient's home Carry out simple home check Undertake essential shopping or accompany to shops safely Collect prescriptions or GP letters Companionship and encouragement to complete daily living tasks independently Telephone assistance, 'check and chat' Assist with light meal and snack preparation Carry out light household tasks e.g. tidying, washing up Prompt with post-discharge exercise plans Top up gas/electricity or help to assist paying bills Advice and information Link in with social activities available and accessible to them This is not an exhaustive list. It is expected that the service provider will add to the list through its own knowledge and experience.
