Haringey CCG Market Event Stroke Support Service
Notice Description
Haringey Clinical Commissioning Group (HCCG) and London Borough of Haringey (LBH) are currently reviewing support services for individuals who have experienced and survived stroke with a view to re commissioning a new service by April 2017. Our joint aim is to commission a support service that promotes independence and enhances the health and wellbeing of individuals who have been affected by a stroke and their carers so that they can participate as full citizens in the community. The service will be focused on supporting individuals who have been affected by a stroke and their carers to access everyday activities such as leisure, education, training and work (paid and unpaid). This is especially important in preventing the more serious psychological consequences of a stroke and in fostering self-reliance and healthy living after stroke. The service will support people and their carers to navigate the services and support available across the system and work in close partnership with a wide a range of statutory and non-statutory agencies in order to ensure improved outcomes and user experience. Key Elements of the Service * Six-month stroke reviews * Stroke club functions * Stroke navigator function * Facilitated and un-facilitated peer support groups We would like to invite you to a market event to help us shape our thinking about the new stroke support service. The workshop will take place on: Date: Wednesday, 25 May 2016 Time: 09:30-12:30 Hours Venue: VENUE TO BE CONFIRMED TO REGISTERED ATENDEES Please RSVP to clinical.procurement@nelcsu.nhs.uk to confirm your attendance including the names and numbers of delegates from your organisation no later than Wednesday 18th May. We reserve the right to limit the number of delegates per agency if there is a high level of interests shown.
