Notice Description
Cambridgeshire County Council and Peterborough City Council currently have a contract regarding the provision of a Care Home Trusted Assessor (CHTA) service across the acute hospitals in Cambridgeshire and Peterborough, plus the John Van Geest Unit in Stamford Hospital in Lincolnshire. Care Home Trusted Assessors (CHTA) provide a service by which adults (excluding people with a Learning Disability) in hospital settings who need to be discharged to a care home are assessed so that they can be placed in the care home setting which best meets their needs
