Notice Description

The London Borough of Tower Hamlets (LBTH) and NHS Tower Hamlets Clinical Commissioning Group (CCG) aims to support and enable people with Dementia to live well as outlined in local and national strategies. The London Borough of Tower Hamlets (LBTH) and NHS Tower Hamlets Clinical Commissioning Group (CCG) are seeking bids from suitable providers to deliver a Dementia Community Inclusion Service from January/February 2021 for a period of 2 years with the possibility of an extension for up to three years. The service delivery is to focus on two main elements: Community based one to one support for people living with dementia and/or their carers to lead fulfilling lives, reducing isolation and loneliness and identifying their own personal talents, strengths and capabilities, and what they can bring to their peers and the wider community. Dementia Friendly Community Network Development role: This role will build upon the established local dementia cafes and singing for the brain group to support the development and delivery of more groups as identified through co-production. This role will work with the wider community including volunteers to develop and invigorate a range of community group opportunities.
