Notice Description

In order to assist this process, the Child Health Information System (CHIS) hosted by County Durham and Darlington Foundation Trust (CDDFT), that holds all local active clinical care records for children in County Durham is required. Using CHIS, CDDFT will support the pilot study by: * Creating a master spreadsheet of all children in the MapMe process and assigning a unique number to each child (Reception and Year 6 children); * Ensuring the spreadsheet is kept up to date in respect of withdrawal of consent, movement out, death; * Entering all screening data for those children within CHIS; * Creating a mail merge document for each schools' results; * Creating, printing and posting letters and outcome sheets to parents / carers of all children in the study; * Six months into the study sending further letters to 50% of programme participants; * After 12 months identifying children who were in the study and notifying them regarding further measurement; * Recording data for the children re-measured in Year 1 and Year 7.
