PIN: Home from Hospital Service
Notice Description
Using the flexibilities of the Better Care Fund, Frimley ICS and Bracknell Forest Council are looking to commission a contract to deliver a Home from Hospital service. This contract will cover eligible patients within Bracknell Forest who are being discharged from hospital. The service will be managed by a coordinator who will liaise with key stakeholders to receive and action referrals. Patients will be supported for a period of six weeks, with visits and telephone calls to help with emotional needs and small practical tasks. The service is not expected to provide personal care. Key aims for this service are to: * Support patients being discharged from hospital * Enable people to live independently in their own home * Improve patient experience * Reduce hospital admission * Assist with hospital discharge * Reduce hospital readmission rates * Supporting patients to be fully sighted on additional community organisations where appropriate The Home from Hospital service will work closely with the Adult Social Care team, OT's and ward staff based in hospitals to help with the timely discharge and unnecessary admittance of patients. The project will also work closely with the Intermediate Care Teams with referrals coming through from Intermediate Care as well. The Council is currently undertaking market engagement prior to a potential procurement process being done by partners under the Better Care Fund. The commissioner reserves the right not to publish any contract notice; not to award the contract in whole or in part; to cancel or amend the tender process and does not bind itself to accept any tender. The Council, and partners, shall not be liable under any circumstances for any costs, charges or expenses incurred by any bidder or prospective bidder in responding to this PIN notice or in taking part in any procurement process that may follow. Additional information: All information pertaining to this PIN is published on the South East Business Portal (SEBP) https://sebp.due-north.com/ Providers must register to ensure they receive updates and information pertaining to this PIN.
