Notice Description

THREE 'Transfer To Assess' Residential EMI beds (as specified in Appendix ONE) are required to complement the existing provision of 'Transfer To Assess' nursing beds, residential beds, add capacity to alleviate seasonal pressures, and facilitate the safe and timely transfer of medically stable patients from an acute setting to a residential setting for further ongoing assessment of their health and social care needs. These THREE beds will be required for a period of SIX MONTHS and will be supported by the multidisciplinary Integrated Discharge Service. The Integrated Discharge Service comprises:
