Notice Description

The Dementia Volunteer Coordinator is not a standalone service. There will be one WTE project worker recruits, manage and works with a team of volunteers. The team of volunteers will encourage older people to be more active and go outside, attend medical appointments, join in with activities, go on outings and help at the Westminster Memory Cafes in the North and South of the Borough with the aims of: * Improving access to primary care and community health services - by supporting the individual to attend appointments * Reducing health inequalities, particularly in deprived/vulnerable communities and empowering local people to improve their own health - by supporting the client to attend exercise classes and dementia cafes to improve their social connections and general wellbeing, as well as physical fitness, where they would otherwise be unable or unlikely to attend unaccompanied. Additional information: A new contract is putting in place for a period of 18 months to afford the newly forming NHS North West London CCG time to agree the approach to commission this service at NWL ICS level in line with national and ICS guidance to ensure smoother, less complicated, non-duplicated service delivery.
