Support provided is not necessarily in the form of care, it can be support provided to help build confidence, re-build daily routines etc. to ensure a person can live a fulfilled life. The aim of the service is to provide an appropriate level of support and intervention to enable a person to live in their own home for as long as possible and retain their independence. Reablement and supporting people to live independently can ensure better outcomes for people and reduce admissions or readmissions to hospital. The contract term is a five year term with an option to extend for a further 2 years.
