Notice Description

In line with the national Transforming Care programme the Southend, Essex and Thurrock (SET) Transforming Care Partnership have agreed over the next 12-18 months to re-procure a number of specialist contracts for Adult provision across health and care services. These services are aimed at people with LD and / or autism at risk of or already demonstrating behaviours that Challenge that can sometimes lead to very poor outcomes, including admission to in-patient health, contact with the Criminal Justice System or poor physical health outcomes. The national service model, announced in 2015, https://www.england.nhs.uk/wp-content/uploads/2015/10/service-model-291015.pdf articulates the value of seamless services across health and care, multi-disciplinary working, outcome based contracts and a shift to community-based preventative health and care services. The purpose of the engagement is not only to support providers to better understand the requirements of the Essex commissioners but also to use the knowledge of the market to design services which are in the best needs of our service users, their families and carers. An initial workshop was held on 4th April 2017 and feedback was received from both social care providers who work with the more challenging and complex individuals with LD and / or autism along with specialist LD healthcare providers. These further workshop sessions will consist of 1:1 sessions with LD health providers only, as this is the first contract that will be re-procured. These will further refine the healthcare component within the wider future system. These sessions will be held on Monday 22nd and Tuesday 23rd May in Chelmsford, Essex. Providers are asked to invite no more than five delegates to attend. Should you wish to attend providers are asked to register on the Bravo portal (link below) and follow the instructions in order to submit an expression of interest. Once registered you will be able to access the slides and notes from the first workshop in April. A bidder pack giving further details for the workshop sessions in May will be published on the Bravo portal for download by the morning of 11th May 2017. Attendees should have the right knowledge and expertise to actively participate in conversations. Any clarification points made at these sessions will be circulated to all participants following the workshop sessions.
