Carers Discharge Support
Notice Description
The Carers Discharge scheme is a short term initiative funded from the ASC Discharge Grant to facilitate hospital discharges up until the end of March 2024. The scheme will make one off payments of up to PS500 to support family/informal carers who are providing support to enable a cared for person to be safely discharged from hospital and for them to be supported at home.
