Notice Description

Wigan has an ongoing commitment to transforming services that focus on better outcomes for individuals whilst ensuring high quality and efficient provision. As part of this a strategic review of Extra Care services in Wigan has recently taken place. The review described the current services as traditional, based on a homecare model which lacked flexibility, responsiveness and community connection. We are looking for an innovative provider that will co-design a new model of Extra Care with the Council, the landlord, residents, and the local community that does not currently exist in the Borough. This new wellbeing model will build on the assets of residents and people in the local community to create a home for life for residents, providing care in line with the principles of "just enough support" to enable them to live the best lives they can, deferring or avoiding the need for hospital admissions or a move to a traditional residential care settings. The detail in the specification describes the support packages as they are currently delivered, however, we want to commission a proactive provider that is able to use their experience and expertise to implement our vision of a wellbeing model of Extra Care, which is flexible, responsive, efficient and deeply connected to the local community.
