Tier 2 Lifestyle Adult Weight Management Service
Notice Description
Wirral Council is seeking to commission a service provider to deliver an accessible tier 2 lifestyle adult weight management service, which delivers improved outcomes and provides excellent value for money for the residents of Wirral. It is intended that service delivery will display the highest levels of innovation, quality and performance and will be financially incentivised through contractual terms. The service will support obese adults (and overweight adults with co-morbidities) to lose weight and learn how to maintain a healthier weight. The service will form an integral part of the local weight management care pathway and aim to impact positively on obesity-related Public Health Outcomes Framework targets. The successful provider will be required to manage and deliver an evidence based service across Wirral however there should be a particular focus on target areas (such as people from more deprived areas) and population groups (i.e. men) to ensure equity of access to the service. The service will advertise and promote itself to referrers such as Health Care Professionals and the general public in order to generate referrals and self-referrals accordingly. Appropriate and individualised advice, help and support will be offered to clients to achieve realistic healthy weight targets through addressing healthy eating, physical activity issues and improving motivation. The service will provide at least three months of intensive support for each client and have the capacity to offer further support for up to 12 months in total. Whilst the majority of client contact will be in community group settings spread across the borough, a fully functioning online resource and linked mobile phone 'app' will be available to support appropriate qualifying adults through their weight management intervention.
