COV - Dementia Community Promoting Independence Service - AWARD
Notice Description
The Council is looking to contract with a provider for the delivery of a flexible, short term promoting independence service, for people living with dementia or cognitive impairment within their own homes. The service will provide Occupational Therapist input to deliver personalised support, along with support staff to aid the person to reach their reablement potential.
