NECS362 Primary Care Dementia Support Service Request for Information - NHS Newcastle and Gateshead CCG - North of England Commissioning Support (NECS) working for and on behalf of NHS Newcastle Gateshead Clinical Commissioning Group (CCG) proposes to engage with the wider market to inform future service model development and/or commissioning intentions for the provision of a Primary Care Dementia Support Service for the population of the CCG area. Newcastle Gateshead Clinical Commissioning Group is interested to hear from providers who could deliver a Dementia support service which would be integrated with Primary Care in Newcastle and Gateshead. This service would be required to demonstrate the successful use of the Dementia Register to reach people diagnosed with dementia who were often otherwise not receiving any support. The primary aim of the service is for every person existing and newly diagnosed with dementia to have access to a named Dementia support worker. The service would be expected to gather feedback for service users to continuously improve the service, report results of the evaluation to commissioners. The budget for this service is PS500,000 for 4 years Registering an Interest for this Market Engagement Exercise The CCG is seeking to obtain input and feedback from provider organisations as part of the development of potential service models for delivery of a Primary Care Dementia Support Service. If your organisation would like to register an interest in responding to this market engagement exercise and obtain a copy of the Request for Information documentation: North of England Commissioning Support (NECS) is utilising an electronic tendering tool (In-Tend) (e-Tendering system) to manage this market engagement exercise and communicate with potential bidders in accordance with Regulation 22 of the Public Contract Regulations 2015 (SI 2015 No 102). It is free to register on In-Tend, which can be accessed at any time of day providing you have a working internet connection. It is intended as an awareness, communication and information gathering exercise offering an opportunity for provider organisations to help inform potential future service models and/or commissioning intentions. Additional information: NECS362 RFI - Newcastle Gateshead CCG - Background - A common comment from people with dementia and their families is that there is no effective, co-ordinated support system available to them, unlike with other illnesses. As a result, they fail to access advice and support that would be of real help and feel that they are bounced around numerous support organisations, having to repeatedly explain their story. There have been numerous pilot projects around the UK over recent years which have addressed this but none so far have achieved long term sustainability. In 2019, 2235 people in Newcastle are registered with dementia while Gateshead register is currently 1881. By 2035 this number will increase by over a third therefore it is important support services are able to meet this increasing demand to ensure people live well with dementia in their own homes for as long as possible. * The service would recruit Dementia Support Workers to review the Dementia Registers in Newcastle and Gateshead practices to contact and offer support to everyone with an existing dementia diagnosis. The focus of the service will be building clear referral pathways with all GPs in Newcastle and Gateshead to provide people with support post-diagnosis. The support worker would ideally work together with GPs/ frailty nurses/ practice nurses /practice navigators etc to provide an integrated service for people with dementia and their carer's. * It is anticipated it will take 2 years to reach everyone on the Dementia Registers through reviewing GP lists. Year 3 and 4 will promote the service through Memory Clinics, which will result in those receiving a new diagnosis of dementia being offered a direct access to a named worker and the support they need. * Dementia Support Service will work with people with dementia to provide person-centred information and support and provide signposting for families and carers. Dementia support workers will strengthen and build upon existing support networks and enable access to new support opportunities. * During year 1 and year 2 the service will contact everyone on the existing Dementia Registers in Newcastle Gateshead, develop a planned approach to undertake annual dementia reviews where agreed with the GP. * It is anticipated that the service will deliver an improved offer during year 3 and 4 to include referrals for everyone newly diagnosed via memory diagnostic pathway.
