Description: The provider will work with stroke survivors to understand their support needs across multiple health and social care services. The contract aims to provide a high quality, non-discriminatory service to: Support stroke survivors to maintain their own health and wellbeing and live as independently as possible in their own homes and community as appropriate. Provide timely and appropriate information, emotional support, practical advice and informal advocacy support for stroke survivors, their families and informal carers. Provide stroke survivors and their families and informal carers with a better understanding of the effects of stroke;
