Stop Smoking Service Level 1 (referral); Level 1 & Level 2 (intermediate smoking cessation); Level 1 & NRT provision (Pharmacist / dispenser); Level 1 & level 2 & NRT provision (Pharmacist / dispenser) & NRT Provision only (Pharmacist / Dispenser) Sexual Health Service (GP) LARC for Contraceptive Purposes Chlamydia Screening & Treatment in Primary Care Delivery of Integrated Sexual Health Services as a SH Sphere Clinic Sexual Health Service (Pharmacy) The delivery of Emergency Hormonal Contraception, in line with local PGD arrangements Screening 15-24 years National Chlamydia Screening Programme Treating positive clients & partners for Chlamydia, in line with local PGD arrangements Advice & referral in relation to longer term needs related to sexual & reproductive H&WB Health Checks (GP) The NHS Health Check systematically offers those aged 40-74 years checks that assess risk of heart disease, stroke, diabetes, kidney disease & vascular dementia.
