TWO LOTS OF TEN 'Transfer To Assess' nursing beds (as specified in Appendix ONE) are required to complement the existing provision of 'Transfer To Assess' nursing beds, add capacity to alleviate seasonal pressures, and facilitate the safe and timely transfer of medically stable patients from an acute setting to a residential setting for further ongoing assessment of their health and social care needs. These TWO LOTS OF TEN beds will be required for a period of SIX WEEKS and will be supported by the multidisciplinary Integrated Discharge Service. The Integrated Discharge Service comprises:
