Notice Description

Surrey County Council is require a Stroke recovery service (SRS), it's designed to support stroke survivors, their families and carers in their recovery by providing tailored support after discharge back to their home or community and into the longer term. Support begins following a stroke and continues on a needs led basis for an average of one year. Following discharge from the service we continue to support people within the community via our network of stroke support groups and volunteers. Following discharge from hospital an assessment of need is offered to identify the support needs of the stroke survivor and carer or family. The Stroke Recovery Coordinator works with the individual to develop a Stroke Recovery Plan identifying the actions needed to reach their desired outcomes. Progress is then reviewed and at each stage their recovery plan and needs will be reviewed.
