North of England Commissioning Support (NECS) working for and on behalf of NHS Bolton Clinical Commissioning Group (CCG) and NHS England: Greater Manchester Health and Social Care Partnership (GMH&SCP)under joint commissioning arrangement are conducting a Request For Information (RFI) exercise to engage with the market and obtain input on the draft service model and specification in respect of an Alternative Provider Medical Service for Bolton currently known as Shanti Medical Centre. Please note that this exercise is not part of any pre-qualification process. It is intended as an awareness, communication and information gathering exercise offering an opportunity for provider organisations to help inform potential future service models and/or commissioning intentions. Additional information: The aim and objective of the service is to deliver essential and additional services to registered patients of the practice currently known as Shanti Medical Centre. It is proposed that the terms and conditions of service, including tariff arrangements, will reflect those contained in General Medical Services (GMS) contracts. There is also the expectation that, in delivering the services outlined at Appendix 2, the successful bidder will do so within the context of the CCG's strategic direction for Primary Care in Bolton as well as the wider strategic redesign of services across the NHS at a national level, both of which place General Practice at the center of system level integrated care provision based around neighborhoods of 30,000 - 50,000 registered populations. Shanti Medical Centre practice is a member of the Rumworth neighborhood in Bolton and it is anticipated that the successful bidder will fully participate in this emerging agenda, building relationships with its fellow member practices and wider workforce and quickly developing a thorough understanding of the neighbourhood and its community assets. Additionally, and under a separate NHS Standard Contract, it is anticipated that the successful bidder will sign up to delivery of the Bolton Quality Contract. Bolton CCG has developed a set of Standards known as the Bolton Quality Contract and its focus is to reduce variation and raise performance across the borough by achieving the following outcomes: * Improved access to General Practice * More responsive access * All practices open 8.00am. - 6.30pm. Monday to Friday * Offer minimum contacts - 75 per 1,000 practice population per week * Access to both male and female GP * Children assessed by a Clinician the same day * Improved health outcomes for the population * Early identification through screening and health checks * Optimum care for those already living with long term conditions * Referrals made at the right time, using evidence based pathways * Reduced health inequalities * The Key Performance Indicators (KPIs) will reflect practice population demographics * Improved support and better care for carers, and people with mental health needs * Reduced variation * KPIs set for individual practices * Every practice will be expected to deliver on all Standards * Support for the CCGs Quality, Innovation, Productivity & Prevention Challenge (QIPP) * Reducing demand on Secondary Care services * Eliminating waste in prescribing * Align with the national Antimicrobial Resistance Strategy to reduce overall prescribing of all antibiotics In 2018/19, each practice commissioned to provide this service in Bolton received the difference between their 'core' (i.e. GMS global sum rate) price per patient and the Bolton Quality Contract minimum offer of PS10
