WSCC JR - Care Home Bed Placements to Facilitate Discharge to Assess with Reablement Care Services
Notice Description
WSCC are tendering for the provision of a maximum 30 beds in the care category of 'Band P' Frail with intensive/complex physical care, needs for use by Social Care teams in the Southern and Mid-Sussex Adult Social Care localities. Contracts will be awarded to up to four Service Providers in accordance with the West Sussex County Council 2007 Care Homes (Terms and Conditions) of Contract and the Supplementary Agreement and will commence on 1st March 2018, for a minimum term of twenty-four months and a maximum term of sixty-months from the commencement date. Services are to be provided in West Sussex to serve Worthing Hospital and The Princess Royal Hospital, Haywards Heath and therefore the location of the service must be within reasonable distance of the respective hospital.
