Notice Description

The Isle of Wight Council ("the Authority") Public Health commissioners are establishing an approved provider list for the delivery of Emergency Hormonal Contraception; Chlamydia Treatment; Supply of Champix; Supply of Nicotine Replacement Therapy; Supervised Consumption and Needle Exchange Services on the Isle of Wight. Potential providers wishing to be admitted on to the approved provider list must complete the Enrolment Questionnaire ("EQ") and be able to demonstrate that they meet all of the minimum criteria listed for the services that they wish to deliver under the approved provider list. Any provider that is able to demonstrate that they meet this criteria will be admitted on to the approved provider list and will be able to carry out these services and recharge the Authority for the activity undertaken.
