Community Navigation Service - Southampton City CCG

Notice Description

Southampton City CCG are seeking the market's feedback to assist in the Commissioning strategy for the intended Community Navigation services. We are looking to commission potentially a single contract for a city wide service covering the service requirements as follows: * Community Navigation for adults with mental health problems * Community Navigation for adults with organic mental illness (i.e. people with a diagnosis of or with suspected dementia, which includes younger adults with early onset dementia) * Generic Community Navigation for all adults. We are also looking for a provider to deliver a strategic leadership function to promote and develop a more joined up offer for Community Navigation working with the services above and other providers across the city, either as part of the above contract or as part of a separate contract for community development. A service specification can be found in the attachments area alongside a Request for Information (RFI) form for completion. This process is designed to help the CCG to form a view of the best way to commission the service and is not the start of the formal competition process.
