Notice Description

NWL CCG requires a service to provide care and support to the residents in Hillingdon offering the following: Wellbeing service, Falls Service, High Intensity Users (HIU), Prevention Service and Take Home and Settle Service. Simplifying the access to the extensive range of preventative services from third sector organisations through a single point of access and referral. Service users include patients with frailty factors including one/more long term conditions and social isolation. Patients in need of a service to be supported home from hospital as there are needs that otherwise would result in an admission. Supporting earlier discharge of patients through offering support, continued support in the home environment to avoid an unnecessary hospital admission. Providing support for older people to manage their health and wellbeing needs. The HIU service will support patients that have a high levels of re-attendance with conditions including: 1) Addiction, 2) Mental Health needs, 3) Medical complaints, 4) Homelessness/housing issues/benefit complaints, 5) self-harm, 6) Loneliness, 7) social issues, 8) vulnerable adults, 9) frequent faller. Key services required include: -Support patients with a frailty factor which, when unsupported, can lead to hospital admissions -Support patients with a range of conditions that might otherwise, without local support, end up becoming frequent attenders -Support patients that are referred to hospital/admitted that require some social support to enable an earlier discharge or discharge avoidance by having some community and social support The CCG currently has a contract in place with H4All to provide the above services. The term of this contract is 12 months, 1 Oct 2021-30 Sept 2022 (value: PS334,548.00) during which time, the CCG will assess the needs of the service users and consider how (if relevant) it needs to: -Secure the needs of the people who use the services. -Improve quality & efficiency in the provision of the services including through the services being provided in an integrated way. NWL CCG is currently of the view that the services described are capable of being provided only by H4All as they are a local service by a local voluntary sector who have the key links with the local community which involves the patients they look after. The organisation is a group of local voluntary sector groups who work in collaboration with one another. The purpose of this notice is to be transparent about the CCG's intentions and to invite anyone interested in delivering the services described above to contact us by 3 March 2022. If it is the case that H4All is the only provider capable of delivering the services described, then the CCG will look to award a further contract to H4All for up to 12 months starting on 1 Oct 22. In 2022 it is anticipated that NWL CCG will be succeeded by an Integrated Care Board (ICB). Any future contract/procurement opportunity may therefore be in the name of ICB.
