Notice Description

The Isle of Wight Clinical Commissioning Group has published the PIN notice as a call for competition. The overall aim of the service is to provide patient initiation of anticoagulation therapy, on-going monitoring and management of patients who require anticoagulation therapy in the community aged 18 years and over. Patients must be registered with a GP practice on the Isle of Wight, including temporary residents. The Objectives of the service are: To Offer clinically safe and cost effective, patient initiation, monitoring and management of patients undergoing oral anticoagulation therapy, to ensure optimum INR control, reduce complications and reduce unnecessary attendance to secondary care. To provide optimal management of INR control and the need for continuation of therapy to be reviewed regularly and discontinued or changed where appropriate. To ensure patients who present with complex anticoagulation issues in the community are referred to secondary care for further advice and guidance. To implement the recommendations of NICE and the NPSA, British Committee for Standards in Haematology (BCSH) guidance on anticoagulation management to demonstrate a safe and risk- managed service. To improve convenience and accessibility to testing facilities for patients, by offering increased choice of location, Patient self-testing, extended hours' availability, domiciliary initiation and management to housebound patients. Provide comprehensive and ongoing education for patients, improving their understanding of their condition and anticoagulation therapy. Respond appropriately and flexibly to developments in national clinical guidance for anticoagulation therapy, updating protocols and Standard Operating Procedures (SOPs) as required. Ensure consistency of care and a common standard across the whole Island to ensure no patient suffers inequalities of care. Ensure minimised risk to patients through nationally accredited training of staff, systems maintenance and contingency planning. The specification documents are attached for your information.
