Notice Description

NHS Nene CCG and NHS Corby CCG are reviewing their current community diabetes multi-disciplinary team (MDT) service. The aim of the community diabetes MDT service is to deliver a best practice care model linking with both primary and secondary care that provides the best possible outcomes for patients. This will be achieved through the early identification of diabetes, provision of education and specialist care for patients outside an acute setting, in order to avoid complications and reduce avoidable hospital admissions and attendances, whilst supporting patients to self-manage their condition effectively. The community diabetes multidisciplinary team (MDT) comprises of diabetes specialist nurses, dieticians, podiatrists, support workers and wellbeing workers with diabetologist leadership. The current contract value is circa PS1 million per annum. Part of the review includes meaningful patient and carer engagement to further understand how the service is working now and how it could be improved in the future. This has included holding a diabetes patient event in Northampton and locality roadshow events in Daventry, Kettering and Wellingborough. Ongoing engagement is planned throughout October and November 2015. This feedback will help determine the priorities for the community MDT service moving forward. As part of this review the CCGs wish to gain a better understanding of the provider market for this type of service. This prior information notice is intended to raise awareness of the CCGs current development work around this service and alert all potential interested providers. When both CCGs have drafted the service specification further information will be published to update all potential and interested service providers. Suppliers are asked to express their interest to this prior information notice via rosalind.carter@ardengemcsu.nhs.uk. The deadline for suppliers to express their interest is 29th October 2015.
