The service must offer a person-centred approach to care and support, and respond to the changing needs of people they support during different chapters of their life. The service should also provide support that acknowledges and works with the needs of families involved and responds to personal, domestic, practical, and housing related support needs. The Service should offer community based rehabilitation to support the discharge care pathway of Service Users (SU) from mental health hospital settings and to meet the assessed aftercare needs of SUs who are subject to Section 117 (S117) of the Mental Health Act 1983 (revised in 2007). Key services required include: * Enable SUs to return to and remain in the community and to function at an optimal level of independence, with appropriate levels of support. * Assesses, addresses and develops the skills required to enable SUs to move on to more independent living. * Support SUs to integrate with the community to reduce social isolation and promote their self-esteem and self-reliance. * Provide a safe and stable accommodation that responds to SUs; personal, domestic, practical, social, cultural, spiritual and housing related support needs. * Support and encourage SUs to achieve their fullest recovery potential whilst empowering them to carry out as many of their activities of daily living themselves and supporting them to have maximum control over how their support is provided. The CCG currently has a contract in place with Certitude - Shirley Gardens to provide the above services (estimated contract annual value PS299,325.31). The current contract expires in 31st March 2022 during which time, the CCG will assess the needs of the service users and consider how (if relevant) it needs to: -Secure the needs of the people who use the services. -Improve the quality of the services. -Improve the efficiency in the provision of the services including through the services being provided in an integrated way. NWL CCG is considering options for making future arrangements for these services beyond April 2022 which may involve looking to tender the opportunity in the future or utilising the DPS for Supported Living. Any capable provider may come forward by 3rd March 2022 to engage with the CCG. If it is the case that Certitude - Shirley Gardens is the only provider capable of delivering the services described, then the CCG will look to award a further contract to Certitude - Shirley Gardens for up to 12 months starting on 1st April 2022. During 2022, it is anticipated that NWL CCG will be succeeded by an Integrated Care Board (ICB). Any future contract and/or procurement opportunity may therefore be in the name of the ICB.
