Surrey County Council are seeking a Life After Stroke Support Service to be provided to o People who are residents of Surrey who have had a stroke, or are affected by stroke. o Carers and families of people who have had a stroke. We require the following: Life After Stroke Support Workers who: o Work on a locality basis (aligned to the 5 CCG and SCC areas) across the whole of Surrey; and who o Each manage a caseload, and undertake a combination of in-reach into hospitals to support early and effective discharge; and o Once the person returns home, visits post discharge to prevent, delay and reduce further deterioration in health and wellbeing and avoid reliance on more costly interventions, avoid readmission to hospital, or avoid admission into residential or nursing care.
