Notice Description

NHS North West London (NHS NW London) requires Provision of Severe Mental Illness - Health Check, vaccination, and screening support services the borough of Hillingdon starting 1st September 2022 to 31st March 2023. The service is to be delivered through a Social Prescriber Link Worker (SPLW) who would work across 6 primary care networks supporting with engagement of SMI patients through outreach work. The SPLW would work closely with GP practices and other providers to identify appropriate patients specifically those from black and minority ethnic groups who need more intensive support for them to engage. The provider will also support with educating practice staff and helping to promote local mental health trainings. Objectives: -Enable patients to be engaged with in an environment that is more familiar and comfortable to them. -Better understand of patient's cultural background and ability to be supported using their mother tongue. -Receive a health check which will enable them to improve their health and wellbeing and signpost to other services as appropriate. -Help improve life expectancy as people with SMI die 15-20 years earlier than the general population. -Help increase the uptake of SMI health checks, update of vaccination, screenings -Enables patients who are disengaged or unable to come to the GP practice to be supported and encouraged to receive their Health Check. -Improve trust between SMI patients and their GP practice. -Help reduce health inequalities. -Help to prevent future health conditions and cost to the system. Key Service Outcomes: -Provide educational briefings. -Provide performance measurement reports on numbers of SMI's. -Provide information on those patients who maybe refusing SMI checks and noting any areas of concern, making safeguarding referrals etc. -Create and co-ordinate all promotional and publicity materials. -Contribute intelligence to key bodies to assist in future service design. Further requirements: -Quarterly reporting on numbers in the following context -Supported access to physical health checks and interventions for SMI patients. -Increase number of people supported to attend and complete -SMI check Develop a personalised model that meets the needs of people with SMI -Support access to SMI health checks and improve the quality of the checks & ongoing interventions. NHS NW London is committed to assess the needs of the service users how (if relevant) it needs to secure the needs of the people who use the services, and improve the quality and efficiency in the provision of the services including through the services being provided in an integrated way. The service is required from 1st Sept 2022 - 31st March 2023. Hillingdon based capable providers may come forward by 17:00 on 22nd August 2022.
