Notice Description

NHS Corby CCGs are seeking to engage with providers and gauge market interest in the potential for procuring an integrated same day and scheduled care service incorporating primary care extended hours and minor injuries and illness service provision. This specification describes the Commissioners' requirement for the provision of services from (during the period of 1st March 2019 - 29th February 2024) (the Contract "Term") from the existing Urgent Care Centre premises at the Willowbrook site, Cottingham Road, Corby, NN17 2UR. The Same Day Access Hub services comprising of the timely treatment of illness or injury in a primary care setting on a scheduled and same day basis. The Same Day Access Hub will include: 1. a clinical navigation service to ensure that patients will receive timely access to a complete episode of care concluding with either advice or an appointment for further assessment or treatment at the unit. It should negate the need for additional signposting or onward referral to other services 2. the same day and unscheduled care provision for illness and injury patients potentially requiring further diagnostics 3. Extended Access for primary care services for NHS Corby CCG, comprising of First Contact Practitioner for Musculoskeletal problems, Mental Health worker and a Health and Wellbeing advisory service supporting primary care services to deliver enhanced access to professionals at the point of care needs. The service will need to be committed to empowering the population to self-care and self-management as well as embracing innovation, through online and digital service provision, in order to make it easier for the public to access rapid health advice and care. The service will assess patients working in partnership with the wider care system across ambulance, primary, community, secondary, mental health, social care and the voluntary sector. It is the intention of commissioners that the provider will deliver the two components of this service through a single contract. The provider will be required to deliver the service in a highly integrated and cohesive service model, thus reducing unnecessary hospital attendance or admissions and enabling optimal patient outcomes. The Commissioner's would like to invite interested providers to express an interest in this service by emailing NELCSU.ProcurementNorthants@nhs.net by no later than 5.00pm on Friday 17th August 2018.
