Medway Council and NHS Medway Clinical Commissioning Group are seeking to procure a high-quality effective service to provide guaranteed homecare capacity during winter 2019-20. This capacity will be dedicated towards avoiding delays in hospital discharge and avoiding conveyance to hospital for people within the community. This service will provide domiciliary care whilst a longer-term care provider is sought. The service is intended to explore the improved outcomes achieved by better patient flow from hospital and urgent response to people within the community.
