Notice Description

The Metropolitan Borough Council of Sandwell is seeking tenders from suitably experienced and competent Providers of a NHS Health Check Service. The NHS Health Check programme aims to prevent heart disease, stroke, type 2 diabetes and kidney disease, and raise awareness of dementia both across the population and within high risk and vulnerable groups. In April 2013, the NHS Health Check became a statutory public health service in England. Local authorities are responsible for making provision to offer an NHS Health Check to eligible individuals aged 40-74 years once every five years. The council therefore require a contractor to identify suitable clients to receive a Health Check and provide an accessible service for clients to attend where they will then receive a Health Check delivered by the contractor. Following the Health Check the client will then receive tailored face-to-face feedback for each individual on their future risk of cardiovascular disease. In addition to the appropriate medical management of risk this will include advice on lifestyle and referral to local lifestyle interventions as appropriate.
