Health and Social Care Connect (HSCC) provides a single access point into adult community health and social care services in East Sussex, enabling adults in need of care and support to access the right service at the right time. The Clinical Nurse Advisor Service will have a central role in assisting people to access information and services in a joined up way through the provision of clinical advice within an integrated HSCC service. As such it will impact and support the delivery of outcomes that focus on improving people's experience of person centred, coordinated care. Key outcomes are that the Service: o improves the experience of adults in need of care and support accessing support. o improves the experience of health and social care professionals referring on to other services. o contributes to health and social care system improvements by bringing together and coordinating access to adult community health and social care services. o supports access to broader statutory, voluntary, community and independent sector services.
